Note: If you are a full-time employee, and you decline health coverage (because you have other health coverage), you will be enrolled in self-pay Long-Term Disability, without the option to decline. Click here for the current year plan rate sheet.

 

Member coverage: 60% of earnings, up to $6,000 per month
