If you participate in the Kaiser or Providence Personal Option Plan options available through the Trust, you may have alternative coverage options including a conversion option and self-payment rights under state continuation laws which may differ from the continuation coverage described here. Generally, you must apply for conversion coverage within strict time limits (often 30 days or less). If you elect continuation coverage, you may elect to participate in another option for which you are eligible during the Trust's annual open enrollment period.
