Life-Threating Illness, Injury and Death

Your Health Plan Helps When You Need It Most

Dealing with catastrophic health issues, such as a life-threating illness or injury, or a death is unfamiliar territory for most us when it occurs. That’s why the Trust steps in to help with services and benefits like those listed below.

Case Management

Case managers provide one-on-one support and help to coordinate care between doctors, explain your benefits, act as your advocate, and help you understand your options and make decisions.

  • Kaiser Permanente Participants: Case managers, as directed by the member’s provider, work within the health care teams to closely monitor high risk members and, in conjunction with the treating physician and other caregivers, intervene so that needed adjustments in the treatment plan are made timely and members learn to access care effectively.
  • Providence Participants: Care Management services are open to all Providence Health Plan members and available at no cost. The registered nurses, social workers, clinical support coordinators and technicians who make up the Providence Care Management team are ready to walk with you step-by-step until your needs are met.
Palliative Care

For patients who are undergoing treatment for a serious or terminal illness, the goal of this care is to provide relief from physical symptoms and emotional stress. It is usually provided by a specially-trained team that works with the patient’s doctors to provide additional support.

Hospice Care

During the final stage of a terminal illness, after treatment has ended, hospice care focuses on controlling pain and other symptoms, so a patient can remain as alert and comfortable as possible.

Advance Health Care Directive

Also called a Living Will, this is a legal document that specifies what actions should be taken for the patient's medical care, if they are no longer able to make decisions for themselves, because of illness or incapacity.

Terminal Illness

If you and your dependents are enrolled in Life, Accidental Death and Dismemberment (AD&D) and Long-Term Disability (LTD) Insurance coverage through The Standard, you have access to these resources and benefits:

  • Waiver of Premium: If you are totally disabled under The Standard's LTD, PERS disability, or Social Security disability, you may be eligible to continue your medical, vision and dental coverage for up to 30 months without premium payments. A life insurance waiver for total disability may be an option through The Standard.
  • Accelerated Benefit: If you become terminally ill, you may be eligible to receive an early payout of up to 80% of your Life Insurance benefit.
  • Life Insurance Conversion: If your group Life Insurance ends, you have the option to “convert” some or all of your current coverage to an individual Whole Life Insurance policy, without providing evidence of insurability (or proof of good health).
  • Online Will Preparation is an easy-to-follow process of answering questions to create a simple will.
  • Life Services Toolkit provides resources for funeral planning, grief support, legal services and more.
Death of a Covered Participant who is Covered by Life, Accidental Death and Dismemberment (AD&D) or Long-Term Disability (LTD)

You or your surviving dependents may also be eligible for these benefits:

  • Survivors Benefit pays benefits to the beneficiary if the covered employee member dies while LTD benefits are payable. If a covered member dies while LTD Benefits are payable, a Survivors Benefit will pay a lump sum equal to 3 times the LTD monthly benefit without reduction by Deductible Income.
  • Family Benefits Package need link: Eligible family members may receive additional financial help for child care, college or career training in the event of the covered member’s death.
  • Continuation Coverage: If the covered member dies, his/her dependents have the option to continue benefit coverage on a self-pay basis.
Grief Support

If you or your family members are grieving the death of a loved one, bereavement support helps survivors cope with grief as they go through the grieving process.
