The following appeal procedures apply to any appeal involving eligibility or benefits not provided by Kaiser or Providence. These are medical and prescription drug benefits under the Trust Early Retiree Indemnity Medical Plan for PAT participants, the Trust Early Retiree Prescription Drug Plan and the Trust Early Retiree Voluntary Dental and Vision Plan.
