The following appeal procedures apply to any appeal involving eligibility or benefits not provided by Kaiser or Providence. These are medical and prescription drug benefits under the Trust Preferred Provider Medical Plan for PAT participants, the Trust Prescription Drug Plan and the Trust Dental and Vision Plan.
