Welcome to the Substitute Teachers Health Benefits Open Enrollment for the 2017/2018 Plan Year

Open Enrollment begins August 15 and ends September 8, 2017

What is Open Enrollment?
This is your only opportunity each year to enroll for Health Benefits, or make changes if you are already enrolled, for the coming Plan Year (October 1, 2017 – September 30, 2018).

IMPORTANT: If you miss the enrollment deadline, you may not be able to enroll for the 2017/2018 Plan Year.


What do You Need to Do?
If you are eligible for Health Benefits, please look for Open Enrollment materials to arrive in your mail. Review the information to understand your choices. Then, complete and return the Health and Welfare Benefits Enrollment/Change Form by September 8, 2017.

  • You must enroll if you are not currently enrolled, and you want to have coverage for the 2017/2018 Plan Year.
  • If you are already enrolled in Substitute Plan coverage and do not wish to make any changes, you do not need to do anything. Your current choices will remain in effect.

The Trust must receive your completed form(s) on or before September 8, 2017. See the table below for all the forms you need to complete open enrollment.
